As parent or legal guardian of the registered participant (the "Participant"), who is under 19 years of age, I confirm that:
- I have read and understood the Release of Liability, Waiver of Claims & Assumption of Risk Agreement and have explained the inherent risks of the Activities to the Participant in terms they can understand.
- I consent to the Participant's enrolment in and participation in the Activities at Gilea Dance Academy.
- On my own behalf, and not on behalf of the Participant, I release, waive, and agree not to bring any claim against Gilea Dance Academy, its owners, directors, instructors, staff, volunteers, and contractors arising from the Participant's participation in the Activities, to the fullest extent a parent/guardian is permitted to do so under British Columbia law.
- I will keep the Studio informed of any change to the Participant's health, medical needs, or emergency contact information.
- I understand and agree to be bound by the Studio Policies.
